Team: Frailty Team Service: Comprehensive Geriatric Assessment Clinic (Pilot)
Length: 10 weeks.
Rate: £31ph
Additional info: Assessment of up to 4 patients in clinic. Looking at falls risk and mobility issues. Follow ups are then completed in patients own home.
Working location: Clinic operates from Stan Ball Centre, Bloxwich, Walsall. Follow up visits will be in patients homes. Needs to be a driver and HCPC registeredBefore You Apply
