Job Description
Locum Practice Nurse – Diabetes Specialist (EMIS Web) – Aylesbury
We are currently recruiting an experienced Locum Practice Nurse for a friendly and well-run GP surgery in Aylesbury. This is a 2-day-per-week role on an ongoing contract (minimum 3 months), with flexible days and start times.
Start Date: ASAP
Contract Length: Minimum 3 months (with potential to extend)
Location: Aylesbury
Working Pattern: 2 days per week (flexible on which days and start times)
Key Responsibilities:

Run diabetes clinics for both ongoing management and newly diagnosed patients
*
Conduct annual reviews and provide structured diabetes care
*
Offer patient education, lifestyle advice, and support in line with QOF and best practice
*
Coordinate with GPs and wider clinical team for continuity of care
Essential Requirements:
*
Practice Nurse experience within primary care
*
Formal qualifications and strong experience in diabetes management
*
Confident running independent diabetes clinics
*
Fully trained and competent in EMIS Web
*
Able to start immediatelyBefore You Apply
