Candidate Responsibilities:
Conduct assessments and reviews to support safe and effective discharge processes from acute and community hospitals.
Collaborate with multidisciplinary teams to ensure the well-being of vulnerable adults.
Maintain accurate and timely case notes and reports.
Participate in regular team meetings and contribute to the development of care plans.
