Job Description
We are looking to recruit a Qualified Social Worker – Discharge to Assess Team with the Social Care industry in Luton.
Responsibilities include:
Working with Vulnerable Adults within a hospital setting
Report writing and making assessments
Implementing and reviewing care packages in the community (Patients home / Care Homes)
Care management
Liaising with other professional bodies
Undertaking Initial & Core Assessments
Making referrals to residential or nursing homes
Experience preferred:
Post qualifying experience of working as a Qualified Social Worker in a Hospital Social Work team within the UK statutory setting.
The Team:
Well-structured organised team, growing their social worker staffing levels.
Minimum Requirements:
SWE Registration
3 year post qualifying experience within UK Social Care industry
Enhanced DBS/CRB (we can obtain for you)
Eligibility to work in the UK
