Key Responsibilities:
*
Care coordination of complex cases under the Care Programme Approach (CPA).
*
Developing co-produced care plans with service users.
*
Undertaking risk assessment and management.
*
Safeguarding adults and ensuring compliance with statutory duties.
*
Completing holistic assessments of mental health and social care needs.
*
Carrying out Care Act assessments and reviews.
*
Engaging with carers and wider support networks.
*
Partnership working with statutory and voluntary organisations.
*
Liaising with inpatient units to support discharge planning and continuity of care.
*
Maintaining accurate records and producing professional reports.
Essential Requirements:
*
Degree/Masters in Social Work and current SWE registration.
*
Experience of working with adults with psychosis and complex needs.
*
Demonstrable experience of Care Act assessments and reviews.
*
Strong organisational skills with the ability to manage time effectively and prioritise workload.
*
Excellent interpersonal skills, with the ability to engage service users from diverse backgrounds.
*
Ability to work as part of a multidisciplinary team.
*
Good knowledge of relevant legislation, policy, and procedures, including Safeguarding, the Mental Health Act, the Care Act, and the Mental Capacity Act.
*
Right to work in the UK.
