This pioneering service supports individuals with multiple complex needs who often fall through the gaps in service provision, helping them to achieve positive change through flexible, connected, and accessible support.

Key Responsibilities:
*
Work within a multi-disciplinary team including Social Workers, Homeless Prevention Officers, Community Support Workers, Domestic Violence & Abuse Workers, Recovery Coordinators, and Peer Support Workers.
*
Discharge the Local Authority’s statutory duties under the Care Act 2014 and Mental Capacity Act 2005.
*
Adopt a curious, creative, and collaborative approach to service provision.
*
Deliver trauma-informed, person-centred support to individuals experiencing multiple disadvantage (homelessness, offending, substance misuse, domestic abuse, and/or mental ill health).
*
Contribute to wider systems change priorities, identifying blocks and barriers in services, and supporting flexible, connected working across Sussex.
*
Maintain accurate and timely case records in line with professional standards and legislation.
Essential Requirements:
*
Degree/Masters in Social Work and SWE registration.
*
Demonstrated commitment to trauma-informed practice and innovative approaches to complex needs.
*
Knowledge and experience of applying the Care Act 2014 and Mental Capacity Act 2005.
*
Ability to work effectively as part of a multi-disciplinary team.
*
Minimum of 2 years post-qualification experience.
*
Right to work in the UK.
