Job Description

My client based in Middlesbrough requires a Recovery Case Worker to work within their Public Health, Recovery Solutions Team. The role is running through to March 2026, with a potential to become permanent. The hourly rate is £15.58 per hour. The purpose of the role is to: Aim to improve the lives and wellbeing of people with multiple needs who require intensive support and may find it difficult to navigate or engage with services, revolve in and out of services or have barriers to accessing services. The Recovery Case Worker role supports people with multiple needs relating to domestic abuse, homelessness and substance misuse, who may also experience additional needs such as re-offending behaviour, mental ill health and/ or physical ill health/ disabilities. The Recovery Caseworker will work flexibly to support people in a way which facilitates engagement. The Recovery Case Worker will use a trauma informed strength-based approach to enable service users to access the services they need and offer a range of evidence-based interventions to help people increase their resilience, gain confidence, acquire the personal and social assets they need to meet their goals, reduce risks and gain/maintain suitable, stable accommodation. The Recovery Case Worker will be working with people who may present with behaviours that challenge, and which may make it difficult for them to establish relationships at any level and may have a history of offending and increased risk of reoffending. We sit within the ACT model (Accessing Change Together) which also encompassing Homelessness and Housing services, Domestic Abuse and Welfare Rights. This new model supports an integrated approach to care for those needing to access multiple services. The Recovery Case Worker will work collaboratively and flexibly across this partnership to ensure access to support is as easy and seamless as possible. This means they must be an effective team-player, experienced in multi-agency working and skilled in positive risk manager. The role combines being responsive, dealing with service users who may present in crisis with being proactive, ensuring effective case management and coordination with service users to improve outcomes. Duties and responsibilities will include: 1. To undertake effective case management and coordination with service users experiencing multiple needs in relation to homelessness, substance misuse and/ or domestic abuse. 2. To assess and engage with service users who are identified by/or referred to MBC Core Service and who ordinarily find it difficult to engage in specialist and /or statutory services or who find maintaining engagement to be challenging. 3. To complete comprehensive assessments, with service users identifying appropriate interventions, services, placements, and packages of support. Present funding requests or updates in relation to a service users' outcomes to an appropriate panel and monitor and review agreed plan/placements/packages so service users can engage effectively and meet their individual needs and goals 4. To deliver a range of evidence-based interventions using a recovery and strengths based approach where solution focused intervention, motivational interviewing and communicating hope are central. 5. To identify, assess, and work therapeutically with risk and trauma. Practice will be creative and underpinned by knowledge of, and skills in evidence-based practice and psychosocial interventions. 6. To plan service delivery contact flexibly to meet individual needs and adopt a proactive approach to engagement so it maximises service accessibility. This includes delivering interventions peripatetically in a wide range of settings and communicating via a range of methods. 7. To work collaboratively with agencies, the service user and their personal support network to collate all relevant information to co-produce personalised person-centred & strength-based plans. Develop effective working relationships and communication with key voluntary and statutory services to embed a more solution-focused response to people with multiple needs. 8. To take the lead professional role where this is required and support colleagues from other agencies, including rapid response to crisis where applicable. 9. To assess risk in collaboration with service users, personal support network and other workers/agencies alongside using own professional judgment and team processes. 10. To provide assertive and persistent support to individuals and agencies to ensure effective engagement to achieve personal goals and aspirations. 11. To regularly review cases to measure outcomes/distance travelled and identify/ overcome triggers/crisis points that may cause disengagement or resistance. 12. To establish, maintain and end purposeful relationships with service users. Gradually reduce the intensity of involvement as the person gains sustained independence and control. Plan exits from service to ensure on-going support from relevant services, building in aftercare and immediate access to services, in the event of crisis. 13. To work with service users to accurately record and maintain their progression, records and plans. Ensure individuals have regular, up to date copies of their support plan and safety plan. 14. To record activity and results, accurately and in a timely manner, to support project evaluation as well as funding and research requirements.
