Job Description

Send Key Worker – Cheshire East Council - CW1 2JZ 3 - 6 Months Contract 37 Hours Per Week We are looking for Send Key Worker based at : Delamere House, CW1 2JZ Key Accountabilities: • Co-ordinate a person centered, outcome focused multi agency approach to assessment, planning and support ensuring that young people, where possible, and parents are treated as equal partners in the assessment and planning process for the co-production of EHCPs. • Act as the main key worker to children and young people allocated to your caseload who require a SEND needs assessment and / or EHCP. • Act as the Named Officer and main point of contact for named setting, schools, colleges and specialist institutions in and out of the Local Authority’s footprint attended by children and young people with SEND who are residents of Cheshire East Council. • Attend multi agency co-production meetings, chairing the meetings as needed. Analyse and evaluate educational, medical, social care and psychological advice and other relevant information and reports collected under the statutory assessment of a child’s or young person’s special educational needs and make balanced, outcome focused service delivery recommendations. • Meet with children and young people, their parents and carers, to discuss and resolve issues relating to SEND support decisions including where necessary formal mediation. Directing families and young people to Independent Advice and Support or advocacy services as needed. • Submit evidence to appropriate Moderation Panels with sufficient information, evidence and costs to enable decision making. • Support the wider SEND team to build a strong inclusive approach in advising and supporting young people and their parents, carers or representatives on matters relating to the provision of appropriate special educational needs support to help them make informed choices through the decision making process. This will require the job holder to act in an advisory capacity in relation to their area of expertise. • Represent the Local Authority at informal and formal mediation and appeals including those at First Tier Tribunal (SEN and Disability). • Review commissioned provision in order to track the child or young person’s progress and outcomes, taking appropriate action to ensure outcomes for individual young people are achieved. • Liaise with non-educational agencies to ensure that appropriate non-educational services are provided. Advise schools and other relevant parties to ensure that they meet the needs of pupils with SEND and make best use of SEND funding. • Ensure resources are allocated, monitored and reviewed to support implementation of SEND support and EHCPs in accordance with guidelines including advice on criteria for accessing and using personal budgets. Monitor, review and analyse relevant data including outcome measures for the purposes of reporting on the young person’s progress against outcomes in plans. Requirements: • Graduate level qualification, or equivalent, in a relevant field (e.g. Education/Social Work/ health related /Educational Psychology or similar NVQ Level 4 - Advice & Guidance/ NVQ Level 4 in learning, development and support services for children, young people and those who care for them (LDSS)/ Post / DIP Career Guidance/Diploma/degree Youth and Community/PGCE/Social Work qualification/equivalent etc.) • Extended experience in/with early years, school, college, Local Authority, health, independent setting in relation to children and young people with special educational needs and disability. • Successful experience of collaborative and partnership working with school staff and partner agencies. • Effective working with families or young people • Effective multi agency working Statutory Assessment (SEND, Child Protection etc.) • Reviewing monitoring and evaluating performance against agreed targets. • Presenting evaluative reports to ley and professional audiences. • Working with and delivering support//careers guidance/advisory services to children /young people and their parents • Effective up to date theory and practice in the field of special educational needs relating to areas of responsibility. • Understand relevant legal frameworks. • Awareness of impact on parents and carers of caring for vulnerable child/young person with high level of need
