What you will do:
Conduct comprehensive assessments of clients' health and social care needs, ensuring a high quality of service
Initiate and coordinate robust support plans that promote independence while taking advantage of community strengths
Identify opportunities for Reablement and enablement and connect clients with these services when needed
Foster independence and wellbeing using a Strength-Based Approach, encouraging clients to assess their needs and risks
Maximise use of community resources and preventative services to better meet individuals' goals
Monitor and review service provision with a sharp focus on delivering positive outcomes
Collaborate with local providers to enhance service quality and effectiveness
Provide detailed advice, information, and support to clients, carers, colleagues, and partner agencies
Engage in safeguarding activities and work as a Safeguarding Officer, ensuring the protection and well-being of vulnerable adults
