Pertemps are working with a supportive and forward-thinking Discharge to Assessment Team in Merseyside to find an experienced Adults Social Worker who can work collaboratively with healthcare professionals to ensure a smooth transition for patients from hospital to home or other appropriate settings. Role Overview  As part of this innovative service, you will play a crucial role in assessing the needs of adults, facilitating discharge planning, and providing ongoing support to individuals and their families.   Key Responsibilities  Conducting comprehensive assessments of patients' needs and circumstances. Developing and implementing person-centered discharge plans in collaboration with multidisciplinary teams. Providing emotional support and guidance to patients and their families during the discharge process. Ensuring compliance with relevant legislation and best practice guidelines. Maintaining accurate case records and documentation in accordance with organisational policies. Candidate Requirements  A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
