Location: Huddersfield Royal Infirmary, Huddersfield, UK
Hourly Rate: £35.00
Contract Type: Full-Time, Ongoing (37 hours per week, Monday to Friday)

What You Will Do:
Undertake Care Act assessments and develop tailored care and support plans to facilitate safe and timely hospital discharges.
Complete Mental Capacity Act assessments and participate in Best Interest decisions as required.
Attend and contribute to post-discharge reviews, including Decision Support Tool (DST) meetings where appropriate.
Address safeguarding concerns and contribute to adult protection work in line with statutory requirements.
Join the team duty rota (after initial familiarisation period), assisting with the management of incoming cases and maintaining efficient team operations.
