You will be a key advocate for human rights, self-determination, and safeguarding vulnerable adults. * Transitions: Supporting young people with disabilities and/or autism in their transition to adulthood from children's services, ensuring a seamless and person-centred preparation for their adult lives. What We Are Looking For: * A recognised professional qualification in Social Work and current registration with Social Work England. * Demonstrable experience in Adult Social Care, preferably within a Learning Disabilities and/or Autism setting. * Excellent working knowledge of relevant UK legislation, including the Care Act 2014, Mental Capacity Act 2005, and Children and Families Act 2014 (in relation to transitions). * A commitment to working in a reflective, anti-oppressive, and rights-based manner. * Strong analytical, decision-making, and high-quality report-writing skills.
