Preparing for Adulthood Social Worker

We are seeking an experienced Social Worker to join our busy Preparing for Adulthood Team on a 6-month fixed-term contract. This is a crucial role supporting young people with complex needs as they transition from Children's Services into adult life. You will be instrumental in ensuring an effective and seamless transition, promoting independence, choice, and control for the people you support.
Key Responsibilities
You will manage a busy caseload and execute a range of specialist statutory duties. The Social Worker will be required to:
* Statutory Assessments: Undertake comprehensive Care Act Assessments, Mental Capacity (MC) Assessments, and apply for Court of Protection (COP) deputyship orders where necessary.
* Safeguarding: Lead on complex adult safeguarding enquiries, investigations, and risk management.
* Transition Planning: Work closely with Children's Services to support an effective transition into adulthood, including coordinating moves from educational placements and hospital settings to supported living.
* Finance and Support: Ensure that we support people with commissioned services and the effective use of Direct Payments.
* Legal Frameworks: Demonstrate a good understanding of the Care Act and the MCA (Mental Capacity Act).
* Case Management: Maintain accurate and timely records, requiring a good understanding of Liquid Logic.

* Hybrid Working: The role offers hybrid working, however, the designated office day is every Thursday to ensure in-person team collaboration and professional support.
About You
