Discharge Planning & Logistics.
Coordinating transfers to skilled nursing facilities, rehab centres, or hospice.
Setting up home health care, physical therapy, or medical equipment (like oxygen or hospital beds).
Linking patients with community programs like "Meals on Wheels" or transportation services.
Clinical Assessment & Intervention.
Evaluating a patient's mental health, support system, and living situation.
Providing immediate support for victims of domestic violence, sexual assault, or child/elder abuse (including mandatory reporting).
Identifying addiction issues and facilitating referrals to detox or recovery programs.
Navigating the bureaucracy of healthcare is a nightmare for most; social workers act as the guides.
Helping patients understand Medicare/Medicaid or apply for "charity care" for unpaid bills.
Assisting with Power of Attorney (POA) and Living Wills so the patient's wishes are respected.
Acting as a bridge between the medical team and the family when there is disagreement over the care plan.Requirements:
