Job Description
Are you an experienced Social Worker with a strong background in mental health, Section 117 aftercare, and exposure to Continuing Healthcare (CHC) assessments? We're working with a forward-thinking local authority looking for an interim professional to join their Adult Social Care team on a fully remote basis.
Key Responsibilities:
Undertake assessments and reviews under the Care Act 2014, with a focus on individuals eligible under Section 117 Mental Health Act.
Collaborate with multi-disciplinary teams to contribute to CHC assessments, ensuring robust and accurate social care input.
Support discharge planning and aftercare arrangements for individuals leaving hospital under the Mental Health Act.
Maintain accurate, person-centred case records and documentation using electronic case management systems.
Ensure compliance with statutory duties and safeguarding responsibilities.
Requirements:
Qualified Social Worker with current Social Work England registration.
Proven experience working within mental health services, ideally within a local authority or NHS context.
Strong working knowledge of Section 117, Care Act, and experience contributing to CHC assessments.
Confident working remotely and independently, with access to a secure and suitable home working environment.
Excellent communication and partnership working skills.
