Halton Borough Council are looking for an experienced and passionate Adult Social Worker to join their Prevention and Wellbeing Team. This is a fantastic opportunity to be part of a proactive and supportive service that focuses on early intervention and enabling people to live healthier, more independent lives.
Key Responsibilities:
Conduct strengths-based assessments and person-centred care planning.
Work collaboratively with health professionals, voluntary sector partners, and families.
Provide advice, information, and support to reduce risk and promote independence.
Ensure Care Act 2014 compliance and promote safeguarding principles.
Support timely discharges from hospital and reduce avoidable admissions.Requirements:
Social Work qualification (BA/MA/DipSW or equivalent).
Current registration with Social Work England.
Experience in adult social care, particularly in preventative or early intervention work.
