Specialist CHC Senior Social Worker
Salary: £45,830 - £51,130 (SW3)
Hybrid Working | Adult Social Care | Permanent
An opportunity has arisen for an experienced practitioner to take on a Specialist CHC Senior Social Worker role within Adult Social Care.
In this senior role, you will lead on Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare (CHC) funding, manage complex cases, and support the resolution of joint funding or dispute issues. You will work closely with internal teams, care providers and local authority partners to ensure appropriate care and support packages are in place, regardless of CHC eligibility outcomes.
You will also contribute to Section 117 reviews, provide expert guidance to colleagues, and play a key role in service improvement, quality assurance and staff development.
Key Responsibilities
Lead and coordinate CHC assessments and reviews (virtual and face-to-face)
Manage a complex clinical caseload from assessment through to resolution
Lead on joint funding arrangements and Section 117 aftercare reviews
Provide specialist advice on CHC, MCA, consent and funding pathways
Work collaboratively with stakeholders to commission and arrange care packages
Maintain accurate case records and contribute to data reporting and performance targets
Support training, induction and continuous improvement within the service
Provide professional oversight, supervision and guidance to social care staffRequirements
Registered with Social Work England (SWE) or the Nursing and Midwifery Council (NMC)
Substantial post-qualification experience in social work or adult nursing
Strong working knowledge of CHC processes, MCA and consent principles
Proven ability to manage complex assessments and make evidence-based decisions
Excellent communication, report writing and IT skills
Able to work autonomously while contributing positively to multidisciplinary work
Full driving licence and access to transportDesirable:
Experience in Section 117 aftercare
Background in mental health, learning disabilities or older adult services
