Specialist Occupational Therapist – Community Neuro & Stroke Team Band 6 | 30 hours per week | Full time Base: Ashton, Wigan & Leigh Division: Community Services Employer: Wrightington, Wigan & Leigh NHS Foundation Trust Are you an enthusiastic and motivated Occupational Therapist looking to develop your specialist skills in neurorehabilitation and stroke care? We have an exciting opportunity for a Band 6 Specialist Occupational Therapist to join our well-established Community Neuro and Stroke Team, providing high-quality, evidence-based therapy to adults across Ashton, Wigan and Leigh. About the Role As part of our friendly, dynamic and forward-thinking interdisciplinary team, you will provide assessment, treatment, and rehabilitation for adults with a wide range of neurological conditions – including acquired brain injury, stroke, and progressive neurological disorders. Working within community settings, you will support patients to achieve their goals, maximise function, and enhance independence. You will manage a defined and varied caseload, undertaking comprehensive, person-centred assessments and developing tailored rehabilitation programmes. Using evidence-based and client-centred approaches, you will work collaboratively with patients, families, carers, and other members of the multidisciplinary team to deliver interventions that make a real difference to people’s lives. Key Responsibilities Provide a high standard of Occupational Therapy assessment and intervention for patients with neurological dysfunction in community and home settings. Take responsibility for a designated caseload, prioritising, planning and organising interventions effectively in line with clinical need. Work collaboratively with patients, carers, and colleagues to establish meaningful rehabilitation goals using recognised goal-setting frameworks. Apply advanced clinical reasoning to formulate accurate diagnoses, prognoses, and treatment plans for patients with complex or chronic presentations. Evaluate and modify treatment approaches to ensure interventions remain effective and evidence based. Contribute to discharge planning, risk assessment and long-term condition management. Provide advice, education, and support to patients, families, and other professionals regarding neurological rehabilitation and self-management strategies. Supervise and support junior staff, therapy assistants, and students on placement. Participate in service development, clinical audit, research, and quality improvement projects. Ensure accurate and timely clinical documentation and adherence to professional and Trust standards. Promote and demonstrate compliance with professional codes of conduct, Trust policies, and clinical governance frameworks. About You You will be a qualified Occupational Therapist with HCPC registration and experience working with adults who have neurological or complex physical conditions. You should demonstrate sound clinical reasoning, excellent communication and interpersonal skills, and the ability to work effectively within an interdisciplinary team. We are looking for someone who is proactive, flexible, and compassionate, with a genuine commitment to providing high-quality, patient-centred care. Experience in community or stroke rehabilitation, and confidence in leading goal-based interventions, will be advantageous. You will also demonstrate the ability to supervise others, contribute to the education of students, and engage in your own ongoing professional development through reflective practice and CPD activities.
