Role Overview: As a Community Outreach Worker, you will enable individuals to live independently and safely in their own homes. You will work closely with a team to ensure the emotional, social, medical, and material needs of the people you support are met. While you will have a monthly rota, this role requires flexibility, including evening, weekend, and sleep-in shifts, sometimes at short notice. Key Responsibilities: - Assist with daily routines such as cooking, shopping, and housework. - Support individuals in making decisions and managing their budgets. - Provide personal care, including bathing and medication administration. - Promote healthy lifestyles and community integration. - Maintain accurate records and ensure compliance with the Care Act 2014 and Care Quality Commission standards. Skills and Experience: - Essential: Clear communication, good listening skills, flexibility, reliability, excellent organisational skills, and the ability to work independently and as part of a team. A clean driving licence with own transport. - Desirable: Experience supporting people with disabilities, a Diploma Level 2/3 in Health & Social Care, and knowledge of the Care Act 2014.
