Job Description

The Team Leader role is a physically and emotionally demanding job. The successful candidate will undertake 6 months induction, working closely with the senior management team. The successful candidate will effectively lead, motivate, role model and evaluate the domiciliary care team, facilitating high standards of person-centred planned care at all times. Duties will be carried out in accordance with standards set out by the Health & Social Care Act 2008 and 2014 and the Care Quality Commission (CQC) Guidance on Essential Standards of Quality & Safety, and other statutory regulations (i.e. Health and Safety). Remuneration will be in line with our transparent pay rates, based on experience and qualifications. Duties will be 40 hours per week, to include alternate weekends. Annual leave is paid at 5.6.weeks per year. There is a requirement to carry out “on call” duties, one week in 4. On-call duties is part of your working scope and the rate of pay reflects this. Christchurch Care celebrated 10 years in business in June 2024 Our business is continually growing. The successful candidate will be joining a happy well established office team of domiciliary administrators and managers. As part of your role, you must be willing and available to carry out domiciliary calls at short notice, on a fair basis, as part of the domiciliary office team. An initial induction period will consist of mainly domiciliary duties, whilst a rapport is built up with the staff and service users. Ability to use initiative and demonstrate rationale for decisions made. * Smart appearance and professional demeanor. Outstanding role model of professional conduct at all times. * Accurate and clear record keeping, with attention to detail * Ability to put the needs of the service users and staff before yours. * Working knowledge of IT. Christchurch Care utilizes Office 365 and Carebeans Rota systems MANAGEMENT OF THE CARE TEAM * Contribute to the recruitment, interview and induction process of new staff. This will include organizing shadow shifts, Line of Sight documentation, booking training, in line with the Care Certificate. Staff will not commence lone working shifts until an enhanced DBS and two positive written references are received. * Carry out medication competency checks * Carry out regular spot checks, ensuring that you act in a way that provides support and relevant information to the staff. * Contribute to monthly team meetings * Carry out staff supervision and appraisals * Provide informal / pastoral support to staff * Book ongoing staff training. Ensure certificates are received and filed in the individual’s personnel file. * Contribute to planning and attend Company staff social events and charity team building events. * Provide ad hoc help with assisting to cover any outstanding shifts. * Develop a working knowledge of Carebeans Rota systems so that you are able to interpret Rota systems during on call duties. * Provision of emergency contact for staff at all times whilst you are on call. * Develop a working knowledge of the skills and strengths of the domiciliary team you are line managing so that you are able to sense when staff are upset or struggling. * Maintain accurate staff records * Develop a specialist interest relevant to your role and become a source of background knowledge, current information and support to the team. Specialist training for the role will be provided. MANAGEMENT OF SERVICE USERS * Carry out assessment of needs for new Service Users and create a person centered care plan. Ensure a risk assessment of each new service user and their premises is carried out. Communicate the care plan to the domiciliary team. Ensure the service user and or next of kin have had the care plan explained to them and have signed the consents to the care. Ensure that service users’ needs are met to a high standard at all times, ensuring service user safety, well-being and provision of social and emotional support. Ensure that the needs of the service users are met in a way that respects the dignity of the individual and promotes independent living. * Ensure care plans are current and valid: Care plans are updated 6 monthly and as necessary. The service user and or family are informed of any changes to the care plan and give written consent to the agreed changes. * Ensure service user files are regularly restocked and out of date information is returned securely to the main office and stored securely in line with GDPR 2018. * Ensure regular contact is maintained with each service users’ family and friends, as directed by each service user. * Ensure the safe administration of service user medication and medication record keeping. * Contribute monthly medication audits, and ensuring the digital MAR charts are maintained and updated when needed. * Carry out regular monitoring visits to the Service Users * Contribute to weekly service user and staff update, which is sent to staff on at the of the week
