Job Description

Job Title: Young Persons Resilience Worker Location: Hertfordshire Rate: £22 - £23 P/H Umbrella Ltd (£20.33 PAYE Inclusive P/H) Responsibilities: Engaging young people with substance use issues to provide support and structured interventions for their health and wellbeing needs. Building therapeutic relationships and delivering personalized care through evidence-based psycho-social interventions, keyworking, group work, and clinical interventions. Conducting comprehensive assessments and developing individual care plans to address substance use and related outcomes. Supporting the development of positive peer support networks and working closely with the Safeguarding Lead to escalate and address concerns effectively. Enhancing links with local children, young people, and family services to provide holistic support. Participating in multi-agency meetings and ensuring timely and accurate data collection and reporting. Delivering training on substance misuse issues and providing information and support to families and carers of young service users. Triaging assessments and making referrals to treatment and community support agencies. Supporting substitute prescribing regimes to improve social functioning and promoting access to treatment for underrepresented communities. Ensuring confidentiality and consent in information sharing while building trusting therapeutic relationships. Assisting individuals in developing protective factors, recovery capital, and achieving holistic outcomes in care plans. Coordinating integrated approaches to support young people in education, employment, or training opportunities. Collaborating with social, health, and criminal justice services to deliver comprehensive care plans. Providing health education on harm minimization, bloodborne viruses, and overdose prevention.
