About the Role
One of my local government clients is seeking a Hospital Liaison Officer to join the Adult Social Care Reablement and Safeguarding team . This role supports the delivery of Section 2 and Section 5 notifications under the Care and Support (Discharge of Hospital Patients) Regulations 2014, ensuring effective communication between hospitals and social care services.
The successful candidate will provide high-quality clerical, administrative and liaison support to ensure safe and timely hospital discharges and accurate record keeping.
Key Responsibilities

Act as the primary point of contact for all hospital referrals and discharge notifications
*
Log and process Section 2 and Section 5 notifications promptly
*
Liaise daily with hospitals to confirm discharge dates and clarify referral information
*
Maintain accurate administrative systems, records, and indexes related to hospital discharge activity
*
Provide statistical information and support reporting requirements
*
Handle telephone and in-person enquiries from professionals, patients, and carers
*
Attend multidisciplinary meetings and case conferences, taking minutes when required
*
Support the maintenance of office systems, equipment and supplies
*
Contribute to continuous improvement within the team’s quality assurance framework
*
Work in line with Council policies including Equal Opportunities, Customer Care, Data Protection and Safeguarding
Requirements
*
NVQ Level 2 or 3
*
Minimum 3 years’ experience as an Administrator or Liaison Officer
*
Strong knowledge of hospital discharge processes and Adult Social Care
*
Experience working to tight deadlines in a busy environment
*
Proficient in IT systems including databases, spreadsheets and email
*
Strong communication and interpersonal skills
*
Ability to manage multiple tasks, prioritise effectively and maintain accuracy under pressure
*
Experience dealing with the public, health professionals and partner agencies
Safeguarding
All staff are expected to safeguard the wellbeing of children and vulnerable adults at all times. A DBS check will be required.
