We are seeking an experienced Personal Advisor to support young people leaving care as they transition to independent adulthood. The successful candidate will work within the Children, Young People & Families division, helping care leavers aged 16–25 to achieve their personal goals in education, employment, housing, health, and wellbeing.
Key Responsibilities

Pathway Planning: Develop, review, and monitor Pathway Plans in partnership with young people, social workers, and partner agencies to ensure aspirations and needs are met.

Multi-Agency Coordination: Build effective relationships with external agencies, families, and carers to deliver joined-up support.
*
Housing & Finance: Assist care leavers in securing and sustaining suitable accommodation and accessing financial entitlements or benefits.
*
Independent Living Skills: Support young people to build essential life skills including budgeting, tenancy management, and accessing local resources.
*
Advocacy: Champion the rights and voices of care leavers, ensuring they are empowered to make informed choices about their futures.
*
Monitoring & Reporting: Maintain accurate records and provide progress updates on allocated cases.
*
Transition Support: Offer consistent guidance as young people move from social worker support into adulthood.
Requirements
*
Experience working with young people or care leavers within a social care, housing, or youth support setting.
*
Strong communication, organisational, and relationship-building skills.
*
Ability to work independently and as part of a multi-agency team.
*
Enhanced DBS for both Children’s and Adults’ Regulated Activity (required).
*
Full UK driving licence and access to own vehicle.
Working arrangement: Hybrid – community visits and office-based meetings.
