Job Description

Social Worker – Children with Disabilities

Achieving for Children is seeking an experienced Social Worker to join the Children with Disabilities Team. This dynamic service supports disabled children and their families across all aspects of social work—from assessment to long-term planning. The team works closely with multi-agency partners and is co-located with colleagues supporting children’s services.

Key Responsibilities:

  * 

    Assess the needs of children with disabilities and their families.

  * 

    Develop and review individualised plans, chair multi-agency meetings, and work in partnership with parents/carers and other professionals.

  * 

    Use a range of social work interventions to engage children, young people, and families, ensuring the child’s voice is central to practice.

  * 

    Support service development projects as directed by the Head of Service.

  * 

    Contribute to mentoring students on placement and newly qualified staff (opportunities available for Practice Educator training).

  * 

    Act as a link worker for a local specialist school.
